    What I found was using older versions of Rufus Bootable USB 'makers' that if I ran the program somehow it COULD see the drive with the dead USB then I would run just a straight up Format with no other boxes checked, an lo & behold the USB came back to life.being recognized again by Windows or Linux (haven't used iOS stuff)..it was my last resort before tossing it..btw I was using Rufus 2.xxx versions.even the 1.xxx worked too.
